I don't think he was taken aback at all. He is the king of the reactionary, paranoid, anti-Obama crowd, he has surely heard this sentiment many times before.
His reaction was one of empathy for the person who made the comment, he made no effort to distance himself from that type of talk.

Even if he suspected that the person making the comment had dementia or some other mental illness, he still needed to address the content of what was said, once it was out there. It could have been done so easily, something like "I do not condone violence of any sort. That kind of talk has no place in a civil debate about politics."
How hard can that be? How obvious to a thinking leader?

He could have done what John McCain did when confronted by an ignortant voter.

He's an Arab!

He could have but he didn't, he's one of those despicable southern Republicans who have taken up the mantra of the once upon a time southern Democrats; same people different labels.

The Washington Post's Plum Line blog says the Secret Service has spoken with the person who asked the question, and determined that "he or she was an 'elderly person' who now regrets making a bad joke."

Unfortunately McCain only went halfway. I remember this and commending him for saying something, except it was the wrong thing. The implication was that all Arabs are untrustworthy, etc. He should've said, "Even if Obama were Arab or Muslim or whatever, it would not matter. Muslims and Arabs are good people. They of course, like us, have their own share of radical idiots...like you, lady."
