The carriers aren't in the columns. The columns are for different phone models.

looks like there are two models for each size (GSM/CDMA)

In theory that might still be Verizon since it's also got CDMA and by law must be sold unlocked.

Bad design with the page - the carrier logos are just thrown in the middle there, the information below is not meant to be attributed to them in columns.

The column information is for the entire page/models: Plus, 6, 5s, 5c
