Two weeks ago, I generated the code from the site recommended by Article Accelerator, posted it here and asked people with smartphones to check that it worked, then I had the panels printed. Is there some further step I should have taken?

Well - for something important, I wouldn't rely on a crowd-sourced answer, I'd prefer to check it myself.

In this particular instance, submitting the proposed QR code to a QR Decoder would probably have been enough - then you would have found out that the image you were thinking of using was NOT going to the City of Chicago website, but to
"h ttp://q-r.to/O6O" (I inserted a space after the "h" so that the phorum doesn't munge up http)

Without reading the original thread - if folks told you that's exactly what the QR code does (goes to toq-r.to), and you were ok with it, then I don't see what anybody else could have done to help you.
