Why no form 5498-SA for 2016? - space-time - 02-12-2017
I have a HSA account and I used to get 2 tax forms:
1099-SA and 5498-SA
This year I don't see the 5498-SA. I am gathering all documents I can and will go to a CPA to do my taxes (I gave up on doing my own taxes now that spouse works and now we have do deduct tuition, child care expenses, etc). Sure, I will ask the CPA why the 5498 form is missing, or maybe I should call my HSA bank (webster bank) and ask what is going on.
Did you guys get such a form for 2016?

5498-SA is for your personal records only, and does not have to be submitted to the IRS:
