Web based ticket support app

Our IT guy is maintaing one server running MS SBS 2003 and also handleing all the employee computer and software problems. He's keeping it all in his head and jobs are falling through the cracks.

We want to install a web based support ticket system employes use to request help, but mainly so he has a record of all requests open and closed. Any good app that would fit the bill? Open source would be nice.

It would also be nice if it had a knowlege base for those recurring problems.
