The UK offers the best care for the dying. The US is in the top 10.
Please note that 'in bed with twin 21 year old blondes' is not one of the option. Nor is 'in my sleep, not screaming like the rest of the people in the car'.

In all seriousness, Mrs. cbelt3 experiences this almost weekly, offering care to the dying at the assisted living home where she works. From a systemic perspective, death with dignity has a lot to do with the people who surround the person who is dying. In her facility's case, they feel that it's critical. Nobody dies alone- when family is not available a vigil is set and someone is with the person at all times, soothing with voice and touch.
