surveymonkey is what we use (paid version). Great features and support.
Could you not afford a month with surveymonkey to do your survey and then cancel? I guarantee that unless your time is truly free in finding another solution, it would be $24 well spent.

Can you cancel after 1 month? The way I read terms of use it's a one year agreement.

Yes, on the pricing page under "select", choose "see monthly plan", and you should see it as $24/mo rather than $17 billed annually

Might not be as highly polished as you'd like, but you can set up forms (including surveys) with Google Apps...
Create a Spreadsheet, and the form, and it'll collect responses in the spreadsheet.
Price is right (free), but like I said, it may not be polished enough for "professional" use.

Depending on your needs you could also explore other options like using an custom surey application. I use
LimeSurvey for a couple of surveys every year and it works well. It's free, open-source, flexible, and offers unlimited responses and unlimited surveys. You do need access to MySQL or SQL Server or Postgres, and it requires PHP. It's been a couple years since I did the initial install for the site I use it for, but recall it being fairly simple as long as you have some basic understanding of databases structure and PHP.
No doubt there are quite a few other options out there as well that work equally well. Surveymonkey is a great option if you want a very easy solution.

You can do a form in Google Docs and it will be free, and consolidate the results into a spreadsheet.

I've done the $20-something Surveymonkey month once a year for a few years and found it easy to cancel. Unfortunately they took a few options away from that level since previous years.
