I used zoomerang at my previous job. It worked well, had nice reporting options and a decent interface.
No experience with surveymonkey so I can't offer a comparison there.

My boss regularly uses SurveyMonkey to conduct surveys about our IT department's performance from regular users. Unfortunatley, I have so thoroughly trained my users not to open emails from unexpected/unusual/unknown senders that they have all, 100%, deleted the emails and immediately emptied the trash. When asked why, they all responded "Who the heck sends and email from "surveymonkey.com" and expects to be taken seriously?!?
There are lots of Survey servers available for Linux and Apache. There are probably similar programs available for OS X too.

I used to work for a company that does online (and other types of) surveys. They build their own.
