Do you have an iPhone/Touch, and must the app be free?
If you do have an iPhone/Touch and don't mind shelling out $4. then check this one out. It's called Pocket First Aid & CPR. It's a great first aid guide as well as a record keeper for all your medical info, such as your doctors, local hospitals, insurance, prescriptions you take, blood type, etc.
You can also add and edit as many notes as you want to each persons record. Super easy, and invaluable.
