I use a blank Word doc. I write the date and note whether or not anything interesting happened that day, and I keep it in reverse chronological order, so all I have to do is open the doc and start typing at the top of the page.
If all you want to do is keep track of how you felt, it doesn't require anything very sophisticated. My health has been deteriorating very rapidly and seriously over the last couple of years, and this system has worked just fine. I also note when I stop or start any new medications, what the dosage is, and how often I take it. I also mention if I have any unusual symptoms that might be related to the drugs. Frankly I rarely use the notes, but I do have the log just in case I want it, and it's come in handy a couple of times.
I kind of discourage people from becoming too "into" tracking this kind of stuff, because it reminds me of the old geezer who got so engrossed in logging his daily activities that he also included information on what he ate, how much, and when, and then he tracked the elimination process of all that food. There's absolutely no reason to get carried away with this - it'll make ya mentally ill!
