Safe to drink the water?

Once you get sick from the germs down there, are you then immune for a time? Or can a subsequent ill-judged sip from a dirty glass make you sick again? Do the natives get sick from the water, or is it just those who haven't been exposed for a lifetime?

And you always hear about this in Mexico - "don't drink the water" but I assume this also applies to central and South America, swaths of Africia, Indonesia, Indian subcontinent... Thailand? Russia? I assume, radioactivity notwithstanding you can drink the water in Japan, and China I've presumed you need mostly worry about pollutants more than infectious diseases?

I'm curious. It's something that's kept me travelling in the "civilized" world.
