I believe they cover only the damage/loss to the vehicle, I don't know if they cover liability (always used my own insurance for that, I confirmed with my insurance that I am covered when I rent a car). The liability is the real issue, since the potential amount can be very large. The loss damage is limited the to the value of the car, which usually is under 20k, unless you rent a sports or luxury car.
