From the videos at the link I posted, there was no hospital for the final removal, it was done in the doctor's office.
I have no ideal how the emergency room visit got billed or paid, I didn't see anything mentioned in what I've read, maybe you can investigate to get answers to those questions you have.
Yes it was a very stupid thing, and she very much regretted it, and suffered. But I will say that there are a lot of people who do stupid things and wind up in the emergency room. Many explain it as "I fell on it".
