Well, that may have to do with people's inability to calculate dose or follow instructions (which weren't great - they give you syringes marked in insulin units and tell you to use XX units, which has nothing to do with the actual dosing recommendations). Also some (not many) counterfeits. And of course a lot of FUD from brand manufacturers.
