Chupa's point. Healthcare workers like my lovely wife are taught to:
Turn on the water.
Soap up.
Scrub for at least the first verse of "old McDonald".
Get paper towel.
Dry hands
turn off water with paper towel
open door with paper towel
toss paper towel in trash as you leave.

I'll open the door with a sleeve or tissue when they don't have paper towels. I prefer those restrooms that just have a light-trap hallway instead of doors.
