I use fastmail and I also had to set up a forwarding rule through godaddy.
The site is a nightmare to figure out. I doubt writing to them will get you a useful response. That's why they're the cheapest.
Anyways, it was such a confusing process that I'm already fuzzy even though it was fairly recent, but you do in fact have to purchase some sort of a "pack" and then use your little credit tokens to buy the e-mail address to use as a forward.
This stuff is included with other registrars (such as DirectNIC which I've been using for years).
To set the alias on teh fastmail end you have to go to "options" and then select "virtual domains" under "settings" to get to the controls you need. Guessing it's going to be trial and error from there.
