You could probably do some experiments to figure it out.
I do know that using normal AT&T phones your get charged for the call to check voice mail (they don't even consider it a Mobile-to-Mobile number).
If you're signed up for online bill access and look at your call usage, do you see calls for when you were checking voice mail? If you're using minutes it will show there.
If your turn off the data access on the iPhone can you still check voice mail? That would probably tell you if it uses data access or airtime for the visual voice mail.
