I know a wedding photographer that happened too - except it was a swimming pool and he was wearing a 510V strobe battery on his waist.

Same guy was shooting an overall from the church balcony, bumped a piece of gear during the middle of the ceremony. It bounced down two flights of wooden stairs, making sure to hit just about every stair - bang, bang, BANG, Bonk, bang, etc...
