My vaccine card says “Stephan” rather than “Stephen”

What a royal PITA. I got my first vaccine five hours away in my hometown, and I guess in theory I am supposed to go back there and ask for a corrected vaccine card.

when did this happen and when did you notice this?

which vaccine did you get? if you took a 2 shot, did you ask at the second visit?

On my card the first date was 5/1/21 and the second date was 5/22/20, she immediately wrote a 1 over the 0

Yes, you should get this corrected - possibly with a phone call. What really matters is if the state has your name correct in their records. I would check the state's records first - thru an online portal?

Not sure it matters. Every vaccine place will vaccinate someone even without ID or a name. Just give you a blank card with the brand, date and lot number so the patient can monitor for any recalls or such

Now that you mention it, I just checked mine and immediately remembered that I was surprised I had to fill out my name myself. Didn't even notice until I got home.

I think you should legally change it to match your card.
