How do I create a writable PDF file from a hard copy? No, I don't have either copy of Acrobat Pro. And how do I turn a PDF file on a website into a writable one?

AFAIK you cannot. You can use Formuate Pro (I believe free from Google code) to add just about any test you want in a PDF file.

Scan them, use Preview's annotations to fill in the blanks and print the resultant document.
