Our library doesn't charge fines for overdue books, but if you keep it long enough (quite a while) they will send a bill for the replacement cost. All is forgiven if the book is brought back, though.

Library I work at only goes to collections on the higher dollar amount accounts. The rest remain on record basically forever and will prevent renewal of accounts. This applies to non-student accounts, students get billed through the bursars office as part of their school bills.
Billed books are refunded all but a $10 billing fee.

I'm chair of the trustees of the Bryant Free Library in Cummington, MA (founded by William Cullen Bryant hisself). We just send Bruno and Harry the Hammer to visit those with delinquent books. No problems here.
