I am also doing this and have found it valuable. I requested my medical records from all my doctors and i got a list of prescriptions from the pharmacy. So I had all my information.
Then I started looking for a way to store it all. I wanted a web based solution so I could access the information from any place. I never did find a good solution though. In the end I decided to keep the information in an excel spreadsheet stored on my idisk. If a good app becomes available in the future I can export to csv and import to the app. I think an ideal solution might be a web based custom database but I don't know of anyone that lets you make your own. If you find something you like please share it here!

Ever seen the Seinfeld episode where Elaine has "Difficult" written on her chart at the Dr.
What's the name of the App? Edit: I thought you were going to give us the name of an App. ![]()

Do you have an iPhone/Touch, and must the app be free?
If you do have an iPhone/Touch and don't mind shelling out $4. then check this one out. It's called Pocket First Aid & CPR. It's a great first aid guide as well as a record keeper for all your medical info, such as your doctors, local hospitals, insurance, prescriptions you take, blood type, etc. You can also add and edit as many notes as you want to each persons record. Super easy, and invaluable. http://itunes.apple.com/us/app/pocket-fi...51164?mt=8
