Juror Qualification Questionnaires and Summonses are mailed to people randomly selected from items such as voter registration lists and licensed motor vehicle operator lists. The questionnaires are used to determine if you are qualified to serve as a juror.

I happen to have some first hand experience with the NYC Jury Duty selection process. About 30+ years after I left the state, a jury duty notice was sent addressed to me at my parent's address. I made some calls trying to figure out why, thinking it might be related to identity theft. They did say that some of what they use is DMV and voter lists, but couldn't tell exactly how I was picked. I called the NY DMV, they were able to find my records and said my last license expired in 1984, and there was no activity after that. I also called the board of elections to check on voter registration, they were able to confirm that I wasn't registered and hadn't voted recently. I think Jimmy Carter was running for president the last time I voted in NY.
