Strange take on that. When I hear it used it's by someone admitting they screwed up and taking responsibility.
On this, we concur.
Almost every time I have heard "my bad" used it was with the attitude of "it was no big deal". So if they were taking responsibility, it was in a very diffident manner, as if it should not matter anyways.

Meh, I have heard it used that way but generally it's because the person is insecure and is hoping others see it as no big deal because they don't want to feel even worse. And it's often used by kids who are surrounded by people who would never take responsibility for anything, not even a simple "my bad". They're at least admitting they did make a mistake.
