It is mostly because there are a lot of dumb people out there that don't know how to use or exercise proper care for their equipment. They think that just because they payed a lot of money for it they can abuse it or use them improperly and it will keep working.
If you take out all the bad reviews from people using them on their laps (of course it is going to fall out, wasn't designed for that) and take out atleast half almost all of the cord problems (can't yank the plugs out by the cable, and you need to provide strain relief when you wrap the cable up, plus your cat shouldn't eat it, and you aren't supposed to twist them in knots, etc.) there really is just a normal mix of reviews.
For the accessories, most of the complains are that they aren't comparable with whatever device they were using. That happens a lot and isn't Apple's fault. The MiniDP sized connector is in use in a lot of Macs with a lot of different options for what can and cannot be done with it.
