What is this (Audio) connector called and where do I get an Adapter?

That's it!

I will see if I can verify with a voltmeter which pins go where, and see if Radio Shack carries what I need to make one...

I've soldered before, and lord willing, I'll solder again... assuming I can find my soldering iron.
