I meant utopia to be funny. Anytime you can find an online community where most people get along well MOST of the time, you're lucky.
I do agree with you on rules (and consistent enforcing of them) being very important. Those are definitely key. Everyone should be subject to the same rules. Anyone who oversees an online community should be able to put personal biases aside and enforce rules consistently. Heck, I've even had "moderate" my own mother before.

I think a lot of the name calling, etc. gets pulled so quickly that, unless you're there while it's happening, you aren't aware it even happened. In a lot of ways, that's as it should be.
I think if someone is having their posts pulled a lot, a mod should step in and PM the offender privately as a warning. I have no idea if that's done here.
